Mail (absentee) Ballot Information

In order to submit a request for an Absentee Ballot, the applicant will need to send me something in writing (can be typed/handwritten) that includes the election that they are requesting the ballot for, the address that they want it sent to, the date of the request and their original signature (doesn't have to be notarized).
